Booking for Yourself
Appointments must be booked for yourself individually. If you need to make an appointment for someone under your care, please ensure you enter the patient's details in the booking form.
Appointment Fee
You agree to pay the fee associated with your appointment. The applicable fee depends on the nature of your appointment (e.g., initial, follow-up), and you will be charged accordingly.
Non-Refundable Policy
Fees are non-refundable except in accordance with our Cancellation Policy.
Payment Timing
Payment processing details will depend on the account type you establish with Botaniqal Clinic.
Additional Costs
You are responsible for the costs of any medication prescribed to you as determined by the relevant dispensing pharmacy.
Confirmation and Adherence
Please carefully note the date and time of your appointment as confirmed in your booking. Incorrect bookings are subject to the Cancellation Policy.
Patient Responsibility
You are responsible for ensuring a quiet, distraction-free environment and a suitable internet connection (if applicable) during your appointment.
More Than 24 Hours' Notice
If you cancel your appointment with at least 24 hours' notice, you will receive a full refund.
Less Than 24 Hours' Notice or No-Show
The full consultation fee is non-refundable if you cancel with less than 24 hours' notice or fail to attend without notification (unless we can fill the appointment you can't make in time).
Rescheduling
If you cannot attend your appointment and notify us at least 12 hours in advance, you will be charged an additional fee of $50 for a rebooking. While we make reasonable efforts to accommodate rescheduling requests, they are subject to availability and cannot be guaranteed.
How to Cancel or Reschedule
Eligibility
You confirm you are 18 years or older, have a documented medical condition for at least 3 months, and are aware of ineligibility criteria (history of bipolar disorder, schizophrenia, psychosis, delusions, or involvement in an opiate treatment program).
Information Sharing
Your health information will be shared as necessary between Botaniqal healthcare professionals, dispensing pharmacies, and as required by the Therapeutic Goods Act 1989.
Complete Medical History
You agree to disclose your full medical background, including conditions, medications, treatments, substance use, and pregnancy status (or intention to become pregnant).
Follow-Up Appointments
Follow-up consultations may be mandatory to ensure your safety and monitor treatment progress.
Natural Therapy Programs
You understand your treatment programs may be subject to change or termination depending on your current situation as determined by the doctor.
Treatment Acknowledgement
You acknowledge that the medication/s you may be prescribed are not registered on the ARTG and are considered unapproved therapeutic goods. Please ensure you review the Consumer Medicine Information of any medication that you may be prescribed. Your consent includes understanding the potential risks and side effects, including but not limited to:
Responsible Use
You agree to strictly follow dosage instructions and refrain from driving, operating machinery, or engaging in dangerous activities while under the influence. As explained in more detail in the consent form you need to sign on your understanding of the legalities regarding this.
Research
Your anonymised treatment outcomes may be used in research (with your permission prior), and you may withdraw consent at any time.
Consent to Data Processing and Overseas Transfer
By providing us with your personal information through our online forms, including those hosted by JotForm, you consent to the collection, use, and disclosure of your information as described in this policy. This includes the transfer and storage of your data on servers located outside of Australia, including in the United States and European Union. JotForm implements advanced security measures to protect data stored on servers located in the United States and European Union.
By agreeing to Botaniqal's Consent for Unapproved Medicines before this consultation, you provide consent to be prescribed an unapproved medicine (if deemed suitable) and acknowledge the associated risks, restrictions, and possible complications outlined below.
You will be asked this again during your initial consultation to confirm that you understand this by saying ‘Yes.’ Your verbal agreement will be documented by the healthcare professional as secondary confirmation of your consent to be prescribed an unapproved medicine.
Understanding of Unapproved Medicines
You understand that the quality, safety, and efficacy of unapproved plant medicine have not been assessed by the Australian government's Therapeutic Goods Administration (TGA). As a result, access to unapproved medicine is only available through a Special Access Scheme (SAS), Authorised Prescriber Scheme (AP), or Clinical Trial pathways under the Therapeutic Goods Act 1989.
Waiver of Claims
You waive and disclaim any rights to claim against Botaniqal Clinic for any potential side effects, adverse effects, or unknown risks related to plant medicine use.
Follow-Up Consultations
You agree to attend regular follow-up consultations over the phone, as directed by your healthcare provider within your treatment plan.
No Recording: You must not record, film, or capture your appointment under any circumstances. If a recording is suspected, Botaniqal reserves the right to cease the appointment, retain the recording device, and delete the recording without compensation.
Botaniqal Clinic and its healthcare professionals are not liable for any decisions or actions you take based on your appointment or treatment. You release Botaniqal Clinic and its providers from any liability arising from your treatment participation.
